Antibiotic Claforan (Cefotaxime)

Cefotaxime is a cephalosporin antibiotic it is used to treat bacterial infections. Its brand name Claforan and introduced by Hoechst-Roussel Pharmaceuticals in 1980.

Mechanism of action(MOA)

Cefotaxime

It works the same as other cephalosporin by binding to penicillin binding proteins (pbps). They block the bacterial cell wall synthesis by attaching themselves to pbps. Therefore killing them in process also known as “autolysis of enzyme” or murein hydrolases. Also due to their action on bacterial cell wall beta-lactam is also called bactericidal. Cefotaxime not just inhibits bacteria but also blocks cyanelles.

Uses of cefotaxim

It is used to treat infections like:

  1. Cns infections – meningitis because of s.pneumoniae , n.meningitidis and h.influenzae
  2. Lower respiratory tract infections – pneumonia because of s.pneumoniae
  3. Intra-abdominal infections- peritonitis
  4. Bone and joints infections – because of streptococcus spp. And s.aureus
  5. Septicaemia – because of e.coli ,klebsiella spp. And s. Aureus
  6. Gyanecological infections – because of endometritis, pelvic inflammatory disease and pelvic cellulitis
  7. Genitourinary system – because of gonorrhoea and urinary tract infections by e.coli and p.mirabilis

These are not first line drugs preferable for these infections.

Dosage and Administration

It is usually administered iv or im to the patient. The adult dosage 1 to 2 g iv every 6 to 8 hours. Maximum dose should not exceed 2g. Please be careful while self administering of drug and slowly inject it in vein. Do not administer more than required dose of drug.

  21. Pruritus
  22. Colitis
  23. Diarrhoea
  24. Tightness in the chest
  25. Dizziness
  26. Fever
  27. Agitation
  28. Irritation
  29. Leg pain
  30. Hallucinations
  31. Loss of appetite
  32. Stiff neck
  33. Vaginal discharge
  34. Clay coloured stools
  35. Epistaxis

Precaution and Warning

Give your full family and personal history to your doctor. Do not spare any details most importantly if you have any allergy to any of the medication.

If you had any surgeries please mention that to your doctor. Also don’t use vaccines while taking cefotaxime as it may not work properly. If you have kidney, stomach , liver or intestinal diseases please inform your doctor . Drug and dosage will be considered accordingly.

Pregnancy and Breast-Feeding

While pregnant women do not have any harmful or toxic effects on the unborn baby while using cefotaxime but they should use it after consulting doctor. While during nursing it does secrete into the breast-milk therefore should not be used as a precaution.

Drug Interaction

Some drugs like rifampin, rifabutin can decrease the effect of hormonal patch, ring or pills therefore should be taken in accordance. Also do not start or stop any medication without consulting doctor. These drugs may interact and can change the course of medicine therefore do not use it with cefotaxime.
